Fanclub-limited DVD-R. Details completely unknown. There seem to be two versions (a red cover and blue cover) but it's unclear if these are different types or perhaps released in different years. Release year is unknown, but the POSIPOCLUB (ポジポックラブ) seems to have closed by June 2013, so this would have to be a previous Halloween; it may have been as early as Halloween 2010.
