This demotape was apparently freely distributed at the band's revival live on 1994-10-01 at Hirakata (枚方) BLOW DOWN. Its existence has come under question because the only place it was ever mentioned was in the biography on KISAKI's official website, which contains some other inaccuracies. However, SHËY≠DË does have a song called DARK SIDE, so it's definitely possible.
The cover shown here comes from a tape that is at least partially fake, which was sold at auction around 2020.
Although the auction seller sold it as the one-track demotape described here, it actually contains the songs CRIME and FUNERAL by Karma (業). These originally come from a fake MP3 rip of DARK SIDE that has been circulating among internet traders for years, which further includes Kunou NI HaTE NI (苦悩二果テニ) by MAZOHYST TRANCE Gakudan Gion (マゾヒストトランス楽団 祇音). It would seem that the auction seller got the fake MP3 rip and decided to craft a physical release around it (for some reason leaving out the third track).
Curiously, a separate auction once sold a different fake demotape, purportedly by the band Levia, which also used the song FUNERAL. In that case, the song title was changed to KELOID.
The jacket of the fake auction demotape is clearly a photocopy of an existing jacket, as wear and tear is visible in the print. So it's possible that the seller was copying a design from a legitimate copy of DARK SIDE, but it's also possible that the seller created a fake jacket and then photocopied it to increase legitimacy. Details are unclear.
