The scans are of the 初回限定盤 version. According to the flyer in the scans, the 初回限定盤 version comes in a VHS case and includes a 16 page booklet. (There is no VHS--it's just a cassette inside a VHS case.) The image here is the case's jacket. (Do we have permission to include the scan of the booklet cover?)

As for the 通常盤 version (this page): according to the flyer, it does not come in a VHS case and does not include a booklet. I've never seen a picture of the 通常盤 version. (Actually, everyone only seems to have 初回限定盤, so I wonder if 通常盤 was cancelled.)
